WHAT

arrange for you to attend our "out of hours" health centre or visit
you at home, as appropriate. This service is only for genuinely urgent
problems which cannot wait.
Appointments and routine enquiries cannot
be made when the surgery is closed.

CHOOSE AND BOOK
In order to offer more choice and flexibility the practice can refer patients via the Choose and Book system.
Following the decision by your doctor to refer you on for further assessment, investigation or treatment, you can choose where you wish to be seen from a number of hospitals or clinics. A list of options will be printed out and information given to you as to how to make your appointment. This will include a unique booking reference number and a password. Please have these to hand when making your appointment.
This information will be ready for you to collect from reception within five days; please make your appointment as soon as you can.
The progress of your referral will be monitored so that your doctor will be able to see the date of your appointment. If you do not contact the booking line to make your appointment within 14 days they will contact you to remind you. If you do not make an appointment after two reminders you will need to be re-referred by your doctor.
